PATIENT INFORMATION/FORMSBRUNSWICK EYE CARE AppointmentsNon emergency appointments are made in advance. Since we feel your time is valuable, we strive to run on schedule. If rescheduling, please give us at least 24 hours notice for the benefit of other patients who may be in need of an examination or emergency care that day. Emergencies Should you experience a medical eye problem that requires urgent care, you may call our office 24 hours a day, 7 days a week at 725-2161. If you lose or damage your eyeglasses or contact lenses, please call us during normal office hours and we will be happy to assist you. Office Fees and Insurance For non-emergency office visits, payment is expected at the time of service. Payment may be made with cash, check, Visa, MasterCard or Discover. We accept most health insurance plans. We also accept Vision Service Plan and AVESIS for eyeglasses and contact lenses.
About Our Eye ExaminationPlease allow 45 to 60 minutes for your comprehensive eye examination which consists of an extensive health history, an internal evaluation of the health of your eyes, including tests for glaucoma, cataracts and other eye diseases as well as a refraction, to determine if you are in need of glasses or contact lenses. Other specialized procedures may be performed based on your specific needs.
